Admission Requirements
- Senior High School Report Card (Form 138-A)
- Certificate of Good Moral Character
- PSA Birth Certificate (photocopy, 2 copies)
- Marriage Certificate PSA, if married (photocopy, 1 copy)
- 3 pcs 2x2 ID Picture
- 2 pcs long brown envelope
- Drug Test Result (BS Criminology)
